It is an application for each household, and the application will be mailed from the city to the head of the household as soon as it is ready. In principle, there are two ways to apply.
世帯単位の申請であり、準備が整い次第、市より世帯主あてに申請書を郵送する予定です。

申請書に振込先口座を記入し、振込先口座の確認書類（申請書名義の通帳の写し、キャッシュカードの写し等）と本人確認書類の写し（運転免許証の写し、マイナンバーカードの写し、健康保険証の写し、年金手帳の写し等）とともに市に郵送していただく方法です。

⑵ Online application method (Available for holders of my number card) After
　inputting the transfer destination account from the minor portal, upload the transfer destination account confirmation documents (notebook in the name of the applicant, cash card, Internet banking image), It is a method of having you apply electronically (identity verification is performed by electronic signature, and identity verification documents are not required). You cannot make a proxy application using the online application method.
⑵ オンライン申請方式（マイナンバーカード所持者が利用可能） マイナポータルから振込先口座を入力した上で、振込先口座の確認書類（申請者名義の手帳、キャッシュカード、インターネットバンキングの画像）をアップロードし、電子申請（電子署名により本人確認を実施し、本人確認書類は不要です。）していただく方法です。

You cannot make a proxy application using the online application method.
オンライン申請方式で、代理申請を行うことはできません。
